Transportation Assistance ProgramThe Transportation Assistance Program was developed to assist individuals who have a medical diagnosis of epilepsy or seizure disorder enjoy the freedom and independence to travel to work, school, stores, doctor appointments, and entertainment. Through this program, the Epilepsy Association of Utah will either assist the individual in completing and submitting forms to apply for disabled bus passes, or provide transportation assistance utilizing the UTA. Currently, this program is not offered in the St. George, Utah area.

This program is available to individuals who are affected by epilepsy or seizure disorder and are unable to obtain a driver’s license, either temporarily or permanently. The individual must be 18 years of age or 16-18 with parental permission. There is a yearly membership fee of $25.00, which also includes discounts and benefits for other programs. If awarded, the individual must be willing to document transportation use of the passes granted and provide a Neurologist-signed Statement of Need. Those interested must complete and submit an application form or request to have one sent by mail.  Individuals who qualify will be granted a pre-paid fare card for use on public transportation up to $250.00 per calendar year per applicant. The EAU will also provide or facilitate access to training on Public programs available for the applicant and their caregiver. Program eligibility will be revisited for qualification every 6 months. Front Runner is not currently covered under this program.
